The first Naruto manga offers a few hints regarding the disease, as described at the end of chapter #401. Sasuke is told by Sasuke how his father was “ravaged from illness and painfully conscious of his imminent death,” and he “forcefully extended his life by taking medication.”

Itachi’s illness was not revealed until the end of the tale. It was suggested during his battle against Sasuke that his ailments were due to excessive use of the Mangekyo Sharean. However, the subsequent confirmation of his illness as the cause of his imminent doom suggests that it is not related to Jutsu.

Why did Itachi die so easily?

Itachi was killed easily due to the fact that He allowed Sasuke to beat him. Itachi’s long-term plan included the creation of Sasuke’s Mangekyo Sharingan by letting him take on the battle. In becoming a part of the Akatsuki, Itachi also set up Sasuke as an hero who defended from the Leaf Village. A little bit of fame was given back in The Uchiha clan.

Even though Sasuke stood at strongest in their battle It’s probable that Itachi was a little hesitant. He has been a victim, fought or escaped from dozens of famous and high-ranking Ninja. If he were fighting someone who has the less powerful version of sharingan, Itachi could have defeated Sasuke If he would.

What kind of disease did Itachi have ?

He was being treated for a respiratory condition that he was diagnosed with shortly after joining the Anbu. While he continued practice and master his sharingan and genjutsu, the illness slowed his physical strength and endurance. That’s why each time Mangekyou sharedan was used it would cause him to be gassed right following. It was never a big deal however, as he never lost a fight , and always moved forward until the very conclusion.

The fight with Sasuke does not be counted as a loss in the record, but he could have killed sasuke in a snap in the event that he truly would. But, itachi wanted to give sasuke the satisfaction of killing him as itachi killed the uchiha clan (with the right motives).

He was aware that it was time to die and decided to join the akatsuki , so it would be much easier for sasuke to locate him and to hate him more. Itachi decided to kill himself by sasukes hand to make amends for his wrongs. This is the reason he left sasuke by himself to become stronger and to pursue his pursuit.

He also learned genjutsu, which is similar to tsukoyomi to maintain his strength by avoiding direct fights. If Itachi wasn’t handicapped by this condition, it’s possible that he would have been the most powerful uchiha.

He was already extremely skilled and was the first uchiha to be a graduate of the academy. Imagine if he were to become in a healthy state and grow stronger (maybe receive EMS).. He could be invincible.

What do you think if Itachi wasn’t sick?

If Itachi wasn’t sick, that things would have been the same way. He knew that Sasuke’s hunt in search of him was never going to end. The illness encouraged them to get together quickly.. He was prescribed “medication” to extend his life to complete his dream of Sasuke. Itachi thought it was the right choice to give up his life to confer new powers on Sasuke and to repair his Uchiha name.

Itachi was diagnosed with a condition. The cause was never clarified however, it was a requirement for to be on medication to extend his life.
